Your Health & Safety


During Confirmation:

  • We will send you an email and text message a few days before your scheduled appointment. You will receive something similar to, "This message is a reminder that Sample has an appointment reserved at Elmwood Dental on Thursday, April 14 at 2:00 PM. Please click one of the options below to confirm your appointment or to request a callback from our office as we do require a response."
  • If we do not receive a confirmation from you, we will call you 1-2 business days before your scheduled appointment. To avoid an open chair fee, we do require you to complete a verbal confirmation or request to reschedule your appointment (at least 24 hours in advance).

Prior to Checking In:

  • We care about you, so please let us know if any of your pertinent information has changed (such as, your address, phone number, email address, dental insurance, or the employer you get your dental insurance through.)
  • We are requiring an updated copy of each patient's dental insurance card. Prior to your appointment, please email a photo of the front and back of your insurance card to patientcare@elmwooddental.comPlease also indicate who the employer is that you get your dental insurance through.

When Checking In:

  • We will ask that you remain in your car when you arrive.
  • Please call our practice at 585-532-5804. Our receptionist will get you virtually checked in and then your provider will call you on your cell phone when they are ready to treat you. If you are deaf, please feel free to check in via text at 585-316-0635. If our receptionist is on the other line, please leave a voicemail with your full name and best cell phone number for us to contact you at.
  • Our receptionist will then send you a text message with a link to complete our COVID-19 questionnaire. This can be done quickly from your car and submitted digitally! We cannot treat any patients without a completed and signed questionnaire.

When you arrive in the office:

We will ask that you leave your personal belongings (coats, purses, etc) in your car or hang them in the vestibule.

  • We are only allowing one patient per operatory. If you have a circumstantial situation that you need an additional family member in the room with you, please call our office in advance so that we can advise you of what protocols will need to be taken to make this safe for you, your family member and our staff.
  • We do require that all patients come into the practice with a mask. If you do not have one, that is OK, we will provide one for you at the front desk.
  • Due to the additional PPE that our staff is wearing to help protect our patients and themselves, we have made some modifications to the temperature in the office. Please feel free to come with a sweater if you feel you will be uncomfortable during your treatment.

Virtual Consultations:
At this time, when we are fully prepared for your virtual consultation, we will reach out to you with a phone call letting you know that the link to your Zoom consultation was emailed.

Open Chair Fee:
Due to the high demand for doctor and hygiene appointments, please know that failing to provide 24 hours' notice to reschedule your appointment or failing to show for a visit will result in an open chair fee.

  • $45    Hygiene Open Chair Fee
  • $100  Doctor Open Chair Fee (per hour)

Estimates and Co-Pays:

We try to provide estimates for any treatment needed but if we have not provided one, please feel free to call us before your appointment and we would be happy to get one ready for you. If you have dental insurance, please remember you are responsible for your co-pay and deductible at the time of treatment. If you do not have dental insurance, please remember that you are responsible to pay for all services rendered the day they are completed.
