This Privacy Policy explains how Elmwood Dental Group, P.C. and Elmwood Dental Group – Implant and Restoration, P.C. (collectively "Elmwood Dental," "we," "us," or "our"), a dental practice located in Rochester, NY, collects, uses, and shares information through elmwooddental.com ("this website"), our forms, our email and text communications, and our digital advertising (collectively, the "Services"). Your Health Information and HIPAA
Elmwood Dental provides dental care and is a covered entity under the Health Insurance Portability and Accountability Act ("HIPAA"). The protected health information ("PHI") we create and maintain in connection with your treatment, including your dental records, treatment history, and billing information, is governed by HIPAA and applicable New York law, and by our separate Notice of Privacy Practices, not by this Privacy Policy.
